Children's Case Management Program

The Children's Case Management program offers case management services to families who have youths diagnosed with developmental disabilities, pervasive development disorder (PDD), or autism. Services are offered to families who reside in Aroostook County as well as the towns of Patten (Penobscot County) and Danforth (Washington County). Children from birth through the age of twenty are eligible for services.
Children's Case Management helps individual families address their own needs with a child and family team. A case manager's primary responsibilities are to coordinate services, to provide advocacy, and to help develop and monitor service plans.
Each family has their own Individual Support/Service Plan (ISP) that identifies a child's concerns. Every ISP includes a strengths and needs assessment, barrier to be resolved, measurable goals and objectives, and resources. A case manager can also help each child and family to compose a safety plan. Case managers coordinate, monitor, and evaluate ISPs on a quarterly basis with the child and family team to determine whether a plan is appropriate and effective. Case managers include a discussion about discharge planning as part of the ISP and at quarterly reviews. Parents are able to decide how long they want case management from the first ISP. Families are able to choose at what time they no longer need or want case management services.
Children and families can benefit from a holistic plan involving all service providers and agencies. This strength-based approach process supports parents as they advocate for their children's needs. As a part of this concept, case managers work with families and service providers to help maintain children in their own communities. Families are the decision makers for their own children. They are able to decide what services they want for their children and what other supports may meet their needs.
Based on the needs of the family, there are two levels of service available. Level One services are for families that have few service needs. Such families may need information, help accessing resources, or guidance in doing their own case management and advocacy. Level Two services are for families with moderate to the most complicated clinical situations and resource needs. These families may have family difficulties that require more help than Level One services can provide. Many families in this situation require the expertise of a skilled clinician.
