Adult Case Management Program

The Adult Case Management Program provides comprehensive case management services to assist adults with developmental disabilities or autism in achieving and maintaining a positive quality of life. Case managers balance relationships with individuals and their various support systems to optimize overall functioning in home and community environments. Utilizing a person-centered planning process, the case manager works with a consumer's team to develop an individualized support plan. The individualized support plan reflects a consumer's vision for personal goals and a desired life strategy. Northern Maine General case managers work with a consumer and their treatment team to ensure provision of appropriate services.
The service is supervised by a Program Coordinator and all clinicians receive required training and education to qualify them to provide case management services. As an integral member of the treatment team, the case manager works with parents/legal guardians, program coordinators, therapists, and other health professionals to assess a consumer's needs in a variety of areas.
